Important information

The organisation with primary responsibility for a Human Research Ethics Committee (HREC) should notify NHMRC of any change of details for the HREC and/or for the organisation(s) which has responsibility for the HREC.

This form is used to notify NHMRC of one or more of the following changes:

  • change of HREC contact details (includes HREC name change)
  • change of HREC contact officer details
  • change of HREC chairperson details
  • change of organisation details
  • addition of a responsible organisation(s).

For changes in HREC membership, registered  HRECs only need to advise of changes to the chairperson (using this form). However, organisations and HRECs that are certified under NHMRC's National Certification Scheme of Institutional Processes related to the Ethical Review of Multi-centre Research  are required to advise of all membership changes (by emailing HREC.admin@nhmrc.gov.au).

Privacy

NHMRC collects personal information from Human Research Ethics Committees (HRECs) as part of the registration process.

This information is collected on contact details for HRECs and for their organisations, as well as details on HREC membership. Without this personal information, a HREC will not be registered, nor will its registration be maintained.

NHMRC uses this information to contact HRECs and their organisations, to confirm that HRECs and organisations agree to the terms of registration, and/or to verify that HRECs’ appointed members meet the requirements of paragraphs 5.1.29–5.1.31 of the National Statement on Ethical Conduct in Human Research. HREC contact email addresses that do not include personal names or are already publically available may be provided to external third parties. However, all other access to this information will be limited to NHMRC staff.

By providing this information, HRECs and organisations acknowledge that it will be stored by NHMRC.

HREC Closure request

Important information

The organisation with primary responsibility for a Human Research Ethics Committee (HREC) should notify the National Health and Medical Research Council (NHMRC) if registration is no longer required.

Where the HREC is responsible for monitoring active research projects, the primary organisation must notify each principal researcher about the HREC's proposed closure and work cooperatively with the researchers to ensure the transfer of these projects to another HREC.

An annual report must be completed for any part of the reporting period (calendar year) that the HREC was in operation and registered with NHMRC.  At the end of the reporting period, NHMRC will provide an annual report form to the organisation for completion.
